Case Management Services
(Community Psychiatric Supportive Treatment Services - CPST)
NEIGHBORING provides Case Management (CPST) services to adults with chronic and persistent mental illness (CPMI). Specialized case managers are available for individuals with co-existing substance abuse problems and CPMI, as well as for individuals involved in the criminal justice system who have CPMI.
Services include:
- Ongoing Assessment of Needs
- Support and Education in Management of Basic Needs and Daily Living Skills (i.e. meal schedule, money management, keeping appointments)
- Assistance with Accessing Natural Support Systems in the Community (i.e. senior centers, activity and hobby clubs)
- Assistance with Linking to Formal Community Services and Systems (i.e. Social Security Administration, LCDJFS, Section 8 Housing)
- Symptom Monitoring
- Coordination and/or Assistance in Crisis Management and Stabilization
- Advocacy and Outreach
- Mental Health Interventions
(i.e. addressing symptoms, behaviors and thought processes that assist in seeking or maintaining education and employment)
